Reporting a hazard/risk is quick and simple with the Mobile App.
Before You Report
Check the on-site Hazard/Risk Register first.
If the issue already exists but isn’t being managed properly, please submit a Safety Observation instead — this helps prevent duplicates and ensures it gets the right attention.
Step 1: Tap the Orange Plus (+) Button
From any screen in the app, tap the orange + icon located at the bottom-right corner of your screen.
This opens a quick menu with several reporting options.
Tap Report Hazard/Risk from the menu.
Step 2: Fill in the Hazard/Risk Details
You’ll now see the hazard/risk reporting form.
Here’s what to fill out:

Name of the hazard/risk

Description explaining the hazard/risk in detail

The Project/Site the hazard/risk applies to
-
The site will auto-fill based on your current location

To change it, tap the site name at the top and choose from the list or search for the correct one

Note: All required fields must be completed before you can submit the report.
Site Managers Only: As a Site Manager, you can set the Initial and Residual Risk Ratings for a hazard/risk directly in the mobile app.
To do this:

Select the appropriate Likelihood of the incident occurring

Choose the potential Consequence of the incident
Once both are selected, the app will automatically calculate and display the Risk Rating (e.g. Very Low, Low, Moderate, High, or Critical). You will need to do this for both the Initial and Residual risk rating.
This step helps determine the level of controls needed to manage the hazard/risk effectively.
Step 3: Add Hazard/Risk Controls
Scroll down to the Controls section.

Tap Add Hazard Control

Enter a Control Name (e.g. “Barricade installed” or “Warning signage added”)

Choose a Level of Control from the dropdown
You can tap the + icon again to add multiple controls if needed.
Step 4: Attach Files or Media (Optional)
Want to include a photo or file?

Tap Add Files

Choose from:
-
Camera – take a photo now
-
Gallery – select an existing image
-
Files – attach a document from your device
-
This is helpful for adding visual context to your report.
Step 5: Submit the Hazard/Risk
Once you’ve filled out all required fields:

Scroll to the bottom of the screen

Tap the Report button

A confirmation message will appear — tap Yes to confirm
You'll then receive a notification that your hazard/risk has been successfully reported, and the administrator/site manager will be advised.
